We are excited that you are developing a new case for a standardized patient (SP) activity.

As you prepare to create this new case, please review the sample template posted in the Faculty Resources section of the HEC Interprofessional Simulation Center website. This document provides an outline of the information that will be required to complete the electronic version. Please DO NOT complete/submit the sample template. Only the electronic version will be accepted.

Standardized Patient Sample Script Template (Reference document) 

Please provide all information requested in the electronic template. At the end of the template, you will be asked to upload other case-related materials (e.g., door sign, checklist, scoring guide, template for students' post-encounter assignment, chart documents, diagnostic results, etc.).

The estimated time needed to complete the electronic template is 3 hours. An extended period of inactivity may result in the template closing without saving some or all entered information. To exit the survey, select "Save & Return Later." DO NOT press the X in the upper right corner of the screen. Doing so will result in the loss of some or all data entered. To return to this survey, you will need both the survey link and your return code.

Please complete the electronic template and submit all case-related materials at least 6 months prior to the activity. The request to implement a new case may not be possible if the required information is not submitted by this deadline.
